A Look into Spravato (esketamine)

This month to accompany our provider spotlight, we have a look into Spravato! Michelle dela Peña, APRN is the leader in Spravato treatment at our office. Below is an interview with Michelle regarding Spravato.

What is Spravato and how does it work?

Spravato is an FDA-approved nasal spray indicated for those who experience treatment- resistant depression. Spravato has been proven effective for adults with Major Depressive Disorder (MDD) who have had inadequate response from 2 or more antidepressant medications. Spravato is self-administered in-office twice weekly. On a specific level, it works with glutamate in the brain to improve depression symptoms.

What has been your experience with Spravato patients?

Spravato has improved patients’ depressive symptoms, thus improving their daily lives at home and work. Of the patients we’ve treated, they had clinically significant reduction in depressive symptoms and a decreased PHQ-9 (depression) score. Only one patient didn’t respond and discontinued treatment after 4 weeks.

Who is eligible for Spravato?

Anyone who has tried 2 or more antidepressants with inadequate response. However, it is not to be used in those with a history of bleeding in the brain, aneurysm, arteriovenous malformation, substance use, psychosis, or pregnancy.

Are there risks to taking Spravato?

Possible side effects include sedation, dissociation, nausea, and increased blood pressure. Patients are monitored in the office for 2 hours after administration of Spravato by trained staff to ensure safety and monitor side effects. Patients are not permitted to drive after the treatment.

How is Spravato different than other medications?

Spravato doesn’t have any known long-lasting side effects like many of the oral medications on the market right now including weight gain, or sexual side effects, etc.

Mindful Moment

With the season changing to winter, it often brings changes which may excite some, and be dreadful to others. No matter if you’re team winter or not, there is always time for a mindful moment. Considering that the temperatures have dropped, what better way to warm up than with a warm drink. No matter if it’s coffee, tea, or hot chocolate, take the time to be present when making and enjoying it. Even if it is as simple as adding water or milk to your favorite instant hot chocolate. By using your senses, you may find appreciation in the process. For example, smelling the coffee before you prepare it, watching the water change color as tea leaves steep, or noticing the texture difference when you take a sip of hot chocolate and encounter a marshmallow. Not only does it warm the body, it warms the soul. So the next time you find yourself in the mood for a hot drink, take the time to savor each moment.
